Create Your Online Patient Account

By creating your own patient account, you’ll have a safe and simple way to access information from your electronic health record.

Safely view, download, print and share your:

  • Lab Results
  • Discharge Instructions
  • Immunizations
  • Medications
  • Allergies
  • Patient Education
  • Continuity of Care Document — an electronic medical care summary that can be shared with your physician

Registering for a Free, Secure Online Patient Account

You may register for an account online, during the admission process, or in person at one of our facilities.

During the Admission Process

  1. If you were admitted to the hospital or emergency department and are at least 18 years of age, you can create an account when you provide your e-mail address to a registration team member.
  2. We will send an email to the address you provide with the personalized information you need to create a secure online account.
  3. You will create your secure online account with a username and password.
  4. You will receive a welcome e-mail with a link to sign in and access your health information.

Request In-person

  1. Go to the Health Information Management department and provide Health Information Management personnel with your e-mail address.
  2. Health Information Management personnel will verify your identity and personal information.
  3. Using the e-mail address you provide, we will send an e-mail with the personalized information you need to create a secure online account.
  4. You will create your secure online account with a username and password.
  5. You will receive a welcome e-mail with a link to sign in and access your health information.
