Resolving Concerns

If you have concerns about your care…

We are committed to providing you with excellent service.  If you feel we aren’t meeting that commitment, please follow this process to resolve your concerns.

  1. First, please talk with the individual involved.  Each person should do all he or she can to resolve the concern.  However, if that doesn’t resolve your issue…
  2.  Please talk with that individual’s supervisor.  After that conversation, if you still have concerns…
  3. Talk with the director of the department about your concern and how we can best resolve it.  If we still haven’t taken care of your concern…
  4. Please call our Patient Liaison at (386) 917-5757 or (386) 804-2815.  We will do all we can to resolve the issue.
