Patient Referrals

A long-term acute care patient is one who needs focused critical care nursing, intensive respiratory services and rehabilitation therapies for his or her recovery. Our services benefit the patient who needs advanced acute medical care for their disease or illness, and are not medically ready to be transferred to a lower level of care, such as a SNF or other subacute facility, the patient's home or a nursing home.

Florida Hospital at Connerton Long Term Acute Care’s primary focus is to restore normal airway/ pulmonary functioning while providing patients with rest and pulmonary muscle reconditioning.

Admissions

To refer a patient to Florida Hospital at Connerton LTAC, the following steps must be taken:

Step 1: The patient must require medically necessary treatments or interventions.

Step 2: The patient must require a continued acute level of care, have a history of failed treatment in a lower level of care or a high rate of recidivism.

Step 3: The referring agent must call our Referral Line at 1-888-276-3705.

Step 4: Our Admissions Coordinator will confirm receipt of referral request and will notify the clinical liaison, who will complete an evaluation, typically the same day.

Step 5: If a patient has been determined eligible, the determination will be phoned to the referring agent and a note will be left in the patient’s chart, with inclusion of specific transfer information.

We facilitate on-site clinical assessments of referrals, and decisions for approval for admission can be expedited. Along with phone referrals, Florida Hospital Connerton accepts referrals by ECIN software.
