The Diabetes
Center
- A comprehensive
model of patient care and education is promoted and includes the
first and largest nationally recognized education program in Central
Florida.
- The mission
is to prevent the development of diabetes, promote the early detection
of diabetes and to reduce the impact of diabetes and diabetes
complications on peoples lives utilizing a team approach.
- Health care
team consists of registered nurses and registered dietitians who
are certified diabetes educators, and exercise specialists, pharmacists,
and social workers.
- Self-Management
services include a comprehensive course for Type 1 and Type 2
diabetes, Senior Program, Gestational Diabetes Management, Nutritional
Counseling for diabetes, hyperlipidemia, osteoporosis, and reactive
hypoglycemia, Insulin Pump Initiation Program, and Exercise Therapy.
- Provide Self-Management
services at seven sites in Central Florida, including Celebration
Health.
- Sponsors
and provides professional education for industry, pharmacists,
nurses, dietitians and counselors.
- Has been
conducting clinical research since 1993 with dedicated personnel.
- Provides
the only dedicated exercise facility and medically supervised
program in Central Florida.
- The Diabetes
Center of Excellence offers an Endocrinology and Diabetes Clinical
Practice at two sites. Nationally known endocrinologists provide
comprehensive services.
- Provides
a dedicated diabetes unit for acute care. Provides support resources
for five monthly support groups and conducts public awareness
and screening activities.
- Established
a Diabetes Foundation in 1995 utilizing a regional board of directors.
- Began its
first Diabetes Outreach Network (DON) in 1994 and expanded to
the Florida Hospital Diabetes Outreach Network, a statewide system
of four DON's and two out-of-state programs in 1999.
- Centralized
education scheduling for six sites.
- Has developed
collaborative partnerships with industry, health departments and
providers.
