Help with Navigating the Hospital Experience

POSTED BY: Heather Fox

Have you ever felt lost when navigating the health care system? Have there been times when you are seeking care for a loved one and just couldn’t seem to get the answers you need? It is hard to navigate the health care system especially when patients are tired and just don’t plain feel good.  That is the reason the position of care coordinator was created at Florida Hospital.  These individuals are patient advocates and their job is to ease the stress for patients and families when receiving care at any of our 8 Central Florida locations. Some of the services provided by care coordinators include:
  • Physician referrals to specialists
  • Answering questions about conditions treated or services provided for an illness
  • Helping schedule follow-up  appointments or testing
  • Helping families access financial counseling for information on insurance coverage
  • Directing patients and families for helpful services such as rehabilitation, nutritional counseling or specialized therapy

“Often patients just need someone on the “inside” of the hospital to help provide answers and guidance. We serve as their extended family members on the hospital side, helping them get the care they need and answer any questions they may have along the way.”  Sara Conn, heart and lung surgery care coordinator.
Florida Hospital offers care coordination in a number of specialties including heart and lung surgery, orthopaedics, cancer, neuroscience and more. For more information and to access our care coordination program, visit us at www.FloridaHospital.com or call us at 407-303-1700.
