About
Your Consultation
During your consultation, a Pain Medicine physician will speak
with you in detail about your pain problem. He will review
your records and perform a limited physical examination.
After
the physician's evaluation, he will discuss an appropriate
treatment plan for you. The treatment plan may vary depending
on the type and duration of your pain. It may include a series
of injections to help reduce your pain.

Arrangements
Please
bring your insurance card and driver's license. It is your
responsibility to pay for deductibles and co-payments as required.
For patients without insurance, payment of the estimated bill
is due prior to delivery of service.
You should
expect to receive a separate bill for the Pain Medicine physician's
services or any other physicians that may provide services
to you (radiologists and psychologists).
